Life Certificate

 

There is no need to take an appointment. Applicants can visit the Consulate on any working day between 9:30 am – 12.30 pm. The service will be on first come first serve basis.  Before, approaching the Consular Office to seek required service, applicant must be ready with complete application form and other required documents, as mentioned in the Consulate website https://www.cgichicago.gov.in/page/display/18/17. This is important to render better and quick service to the applicants.

Please apply strictly as per your jurisdiction for which you have verifiable proof of address.  Following is the Consular jurisdiction of Consulate General of India, Chicago: Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, North Dakota, South Dakota, and Wisconsin.

The Consulate reserves the right to reject applications which are objectionable or contrary to the Rules. The Consulate has no agents for rending this service and applicants should approach the Consulate directly, either in person or through postal mail.

Kindly bring photocopies of the required documents as there is no facility of photocopy at the Consulate.

 

Note: In addition to pension beneficiaries, there is a regular inflow of applicants (both Indian and US passport holders) who want Life Certificate, presumably for reasons other than pension benefits. We may impose appropriate fee for such applicants.

 

Applying  in Person (Documents required):

 

1.

Download the Miscellaneous Application form and fill it up carefully. Each applicant is required to fill up separate Miscellaneous Application form and affix his/her photograph on the same.

2.

Applicant who is seeking LIFE CERTIFICATE needs to appear in person at the Consulate.

3.

Original and a photocopy of the Life Certificate, which has been provided by the Pension Authorities in India. In the absence of this document, Consulate will issue Life Certificate as per its own format.(Download the form in Consulate’s format.  Life Certificate form and fill it up carefully.)

4.

Applicant’s current valid passport in original and its self-attested photocopy. Those having Indian passport may provide original and self-attested photocopies of the first five pages and last two pages of the passport.

5.

Original and self-attested photocopy of the documents showing valid US Visa Status(any one of the following) :

Photocopy of the page containing visa on passport (H1B, H4 etc) and the copy of I-94, Clear Photocopy of Green Card, Employment Authorization Document (Work Permit), I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being taken to regularize status).

6.

Original and self-attested photocopy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.   

Note: Bank/credit card/mobile phone statements are not accepted as residence proof. 

7.

Carry all original supporting documents for presentation to the Consular Officer. Photocopies of these documents may be attached with the application.  Ensure that photocopies of these documents are legible. 

8.

Fee:  Gratis (no fee). However each applicant applying for Life Certificate has to give a money order/ cashiers check/ cash of US $ 2 (Two) drawn in favor of Consulate General of India, Chicago for Indian Community Welfare Fee (ICWF) w.e.f Septmeber 1, 2017. Fee in cash is acceptable only for personal submission but not through postal mail.

Applicants are requested to come with exact fee amount for efficient service.

PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

9.

If you want to receive the Life Certificate back through postal mail, kindly provide a track able pre-paid return mailing envelope (USPS ‘Express mail’ OR FedEx Express OR UPS) to the Consulate at the given address:

10.

Processing Time: Application will be processed and handed over to applicant on the same day.

 

Applying by Post (Documents required):

1.

Download the Miscellaneous Application form and fill it up carefully. Each applicant is required to fill up separate Miscellaneous Application form and affix his/her photograph on the same.

2.

For postal applications, a recent (no later than 10 days) certificate from a registered medical practitioner, certifying that applicant (with name) has been examined (with date)and unable to travel, by the medical practitioner.

3.

Notarized Original and a photocopy of the Life Certificate, which has been provided by the Pension Authorities in India.

In the absence of this document, Consulate will issue Life Certificate as per its own format.(Download the form in Consulate’s format  Life Certificate form  fill it up carefully and send notarized original and photo copy.) 

4.

Applicant’s current valid passport in original and its photocopy. Those having Indian passport may provide original and photocopies of the first five pages and last two pages of the passport.

5.
