Permit to Carry Mortal Remains / Ashes

  • There is no need to take an appointment. Applicants could visit The Consulate on any working day between 9:30 AM - 12:30 PM. The service will be on first come first serve basis.
  • Please also note that notarization is not required as you will be submitting original documents.

Documents required for issue of permit to carry Dead Body:

1. Miscellaneous form to be filled by the Applicant  : Download and complete the Miscellaneous Application form

2. Copy of Passport, Visa , ID of the applicant

3. Death Certificate of the deceased in original, along with a copy there of .

4. Letter from Doctor or County regarding non communicable disease in r/o the deceased in original along with a copy thereof

5. Letter from Funeral Home concened regarding body embalming casket in original, along with copy thereof

6. Flight Details

7. Passport in original in r/o the deceased as well as photocopy thereof

Documents required for issue of permit to carry ashes:

1. Copy of Passport, Visa , ID, Address proof of the applicant

2. Death Certificate in original, along with a copy thereof

3. Cremation Certificate in original, along with a copy thereof

4. Passport in original in r/o the deceased as well as photocopy thereof

Fee:

The following fees are based on the citizenship of the deceased.

For Indian passport holder: Gratis (No Fee).

For non-Indian passport holder, if carrying ashes: $42 .If you want to receive the permit back through mail, send a traceable pre-paid return mailing envelope (USPS 'Express Mail' or FEDEX etc.) 

For non-Indian passport holder, if carrying mortal remains: $ 62. If you want to receive the permit back through mail, send a traceable pre-paid return mailing envelope (USPS 'Express Mail' or FEDEX etc.) 
If you would like to receive the permit back through mail, send a traceable pre-paid return mailing envelope (USPS 'Express Mail' or FEDEX etc.) 

Fees should be paid through Money order or Cashier's cheque drawn in favor of the 'Consulate General of India, Chicago.’ Cash is accepted only if applying in person. Personal cheques, debit cards, credit cards or other banking instruments are not accepted.
